In addition to the standard annual membership that includes unlimited visits, minimal wait time and 24-hour access to the doctor, this includes quarterly home visits and limited accompaniment to specialist visits. Ideal for those patients with more complex medical circumstances.
Member Services
Includes an in depth discussion regarding past and current medical history, family history, social history and medication use. Physician will compile all information together with the patient and counsel on proper medical care.
Will order bloodwork as necessary and will include a follow up phone call to review. (Bloodwork will be billed directly to patient’s insurance)
